VMAT and TomoTherapy  TBI, TMI and TMLI – whole-body irradiation in the 21st century

Radiotherapy during myeloablative conditioning for stem cell transplantation, highly conformal planning techniques

Myeloablative Total Body Irradiation (TBI) has since long been a cornerstone of the conditioning for hematopoietic stem cell transplantation (HSCT). Currently, use of TBI is mainly indicated for high-risk hematologic malignancy with allogeneic HSCT. The aspiration to reduce acute and long-term effects has induced changes in HSCT and TBI protocols over time. Institutions have varying site-specific setups and techniques for conventional TBI. An increasing number of centers are implementing highly conformal techniques with VMAT or TomoTherapy. Total Marrow Irradiation (TMI), Total Lymphoid Irradiation (TLI), and Total Marrow and Lymphoid Irradiation (TMLI) target the bone marrow and/or lymphoid volume while reducing doses in the remainder of the body, thereby optimizing the balance between treatment outcome and toxicities.

In the 2025 VMAT and Tomotherapy TBI Teaching Symposium, international experts who have cooperated in the establishment of protocols and scientific publications will take the floor to give presentations regarding current and future perspectives for highly conformal radiotherapy techniques during myeloablative conditioning for HSCT in children and adults. The program will cover diverse aspects of implementation of VMAT and TomoTherapy TBI, TMI and TMLI treatments. There will be breakout sessions for physicists, radiotherapy technologists and radiation oncologists, where we will provide an in-depth focus on technical and clinical aspects of VMAT and TomoTherapy planning and treatment for TBI, TMI and TMLI, for different treatment planning systems and treatment machines.

Moreover, there will be ample room for connection and discussion with international colleagues to exchange experiences during the daytime program and social gatherings on June 16 and 17.

The TBI symposium is of interest for radiation oncologists, medical physicists, radiotherapy technologists, hematologists and anybody with an interest in TBI, TMI and TMLI. We hope to welcome you in Utrecht in June 2025!
